From the 1st April, referrals to the service should be made to CDDFT using the referral form linked below and returning it to:  cddft.childrenssalt@nhs.net This form will be made available on the CDDFT website from April 1st on this page: Children’s speech and language therapy :: County Durham and Darlington NHS Foundation Trust
