FOR A CHANCE TO GRAB A FUN CHILD / YOUNG PERSON WELLBEING PACK PLEASE DO OUR SURVEY – SURVEY NOW CLOSED.

Now that we are starting to return to a more normal way of life, Making Changes Together (MCT) Durham would like to get an idea of how children and young people age 0 – 25 with Special educational needs and or disabilities are coping/coped with this.

We would appreciate if you would take the time to complete this survey so that as a Parent Carer Forum, we can see what is working, what is not working and identify any needs in County Durham.

**To be with a chance of winning a Wellbeing bag for your child/young person please fill in all the boxes at bottom of survey** Bags fill be sent via post or delivered depending on location.

This is only open to families on County Durham ( you get council tax bill from Durham County Council)

Survey closes on 25th March 2022.
